What are Eyelash Extensions?

Eyelash extensions are semi-permanent lashes made from a synthetic material (PBT). Each synthetic lash is individually applied to your natural lash (NL) using a special formulated adhesive. It is placed near the lash line but not to the skin. This takes time, careful attention to detail and precision. It is a beauty enhancing service and you don’t need to wear mascara. The length of the lash extensions can be adjusted from short to long and the curl can be more natural or strong to suit your preferences. The extensions are so natural looking, it will blend seamlessly with your own lashes and it is difficult to tell up close that you even have extensions on.

How long do the eyelash extensions last?

The lash extensions can last up to 4 weeks with proper care and maintenance. However, the extensions are not permanent as we lose 2-5 natural lashes everyday and the adhesive naturally deteriorates. When our natural lashes fall out, the extensions attached to the lash will fall off as a new natural lash gradually grows out and takes its place. Factors such as your NL growth cycle, lifestyle choices and general after care processes will also affect the longevity of your lashes. Sleeping on your face, extreme weather changes, exercising regularly or using a sauna may cause the adhesive to break down faster or crimp the extensions and you may require more frequent infills. Please seek professional help from a lash technician and do not attempt to remove the last extensions on your own as you may damage your NL in the process if removed in an incorrect manner. Artistry of Lashes will not be held liable for any damage you may cause by trying to remove the eyelash extension on your own.

Do the Eyelash extensions damage my natural eyelashes?

No, not at all! The adhesive is formulated to create a strong bond and made specifically for human lashes. It dries softly on the lashes, allowing the lashes to stay flexible and bouncy just like your own natural lashes.

What happens if a lash extension become loose or is dislodged?

Do not pull on the lash. Please contact your lash technician to make an appointment to have them removed or replaced in order to avoid damage to your NL. As your NL falls out due to the growth cycle, you will need to infills when new lashes come through. At first you may find that you need an infill every 2 weeks. As you become more accustomed to the extensions and with a little practice maintaining the lashes, the time between infills can be extended to 3-4 weeks.

I agree to the following eyelash extension follow up and maintenance

*No mascara *No oil based products makeup or facial cleansers *No tinting or perming/curling of the eyelash extensions *No use of sauna's hot yoga extensive exercises that will cause excessive sweating round the eye area

I am informing the lash artist of any following conditions including current use of

* oil base sunscreen moisturisers used round the eyes * eye drops of any kind prescription or over the counter *any allergies or sensitivity *history of any eye or tear duct infections *history of chemotherapy in the last 12 mths *other medical conditions which would prohibit or compromise placement and retention of eyelash extensions ie extensive loss of hair

Have you had eye surgery in the last 1 month? Do you wear contact lenses?

I understand that infills are done round the 2-3 week mark, I consent that infills will only be done when there is 40% or more of my lash extensions remaining

I do or do not currently have any eyelashes extensions on from a previous salon. And I understand if I do have existing eyelashes on and have an infill done by Artistry of Lashes any premature fall outs due to the different products used has been explained to me

How much makeup do you wear?

I consent to the 24hr cancellation policy and booking deposit required for any lash extension services

I understand that this procedure requires single synthetic eyelashes to be applied with adhesive to my own natural eyelashes. I understand that it is my responsibility to keep my eyes closed and keep still during the entire procedure, until my eyelash technician addresses me to open my eyes. I understand that some risks of this procedure may be but not limited to eye redness and irritation. The fumes from the adhesive may cause my eyes to tear up if I open my eyes. I agree to disclose any allergies that I may have with latex, surgical tapes, cyanoacrylate, vaseline, etc. I understand that I am required to follow the eyelash extension care in order to maintain the life of these extensions. I agree that by reading and signing this consent form, I release Artistry of Lashes from any claims or damages of any nature. Futher more, I understand that in rare circumtances a reaction may occur from this procedure. I acknowledge Artistry of Lashes cannot be held responsible if such a reactions occur. If I have a problem or have a reaction I will email within 24hrs of my treatment and I should start to use antihistamine tablet. It is safe to do so if I am not allergic to them, and antiseptic cream may be used onto the affected area to alleviate the problem and seek medical attention. I am of sound mind and fully capable of executing this waiver for myself. I give Artistry of Lashes permission to show my before and after photos of eyelashes to other potential clients. I have read and completed the Eyelash Extensions Artistry of Lashes form in its entirety and have answered everything to the best of my ability. I have been informed of potentially harmful or negative effects that may be caused by the application and/or removal of Eyelash Extensions. I confirm and agree that I wish to engage the services of Artistry of Lashes to apply eyelash extensions.
